The aim is to provide them with the skills and knowledge to manage their condition with less admissions to hospital.
“There is no short-term fix for people living with chronic lung disease. It is a long-term condition and as such we need to engage with patients in planning their care and supporting them to self-care. This new service will provide personalised care, as well as support self-management to patients. They will receive disease-specific education and training to become more active and involved in their care planning.”
